Job Summary
Conduct detailed patient evaluations by reviewing history, performing assessments, and formulating tailored recommendations. Engage patients and families to collaboratively develop goal-oriented treatment plans while implementing evidence-based protocols. Consult with multidisciplinary teams to ensure holistic care, monitor condition changes, and facilitate discharge planning with appropriate follow-up. Maintain accurate medical records including progress notes and discharge summaries. Manage time effectively during therapy sessions to meet productivity expectations while promoting community outreach initiatives. Work with diverse populations ranging from post-surgical patients to those with chronic pain and mobility impairments.
Required Qualifications
- Minimum of a Bachelor's degree in Physical Therapy from an accredited program
- Valid State Licensure in Physical Therapy as required for practice in Florida
- Current certification in Basic Life Support (BLS) for Healthcare Providers through the American Heart Association
Desired Qualifications
- While a minimum of two years of experience as a Physical Therapist is preferred, new graduates with exceptional potential will also be considered
- Experience working in outpatient rehabilitation settings is a plus
