Job Summary
Conduct thorough examinations and evaluations of patients to assess functional limitations, then develop and implement customized treatment plans based on unique needs and rehabilitation goals. Provide consultation and care coordination with other healthcare professionals, while regularly evaluating treatment effectiveness to adjust protocols as necessary. Document all interactions, findings, and treatments accurately in written and electronic formats, and supervise auxiliary staff including PTAs and students. Engage in community outreach, quality improvement activities, and program development to enhance service delivery, working in partnership with patients and families to create effective discharge plans.
Required Qualifications
- A Bachelor's Degree in Physical Therapy
- A valid State License in Physical Therapy
- Basic Life Support (BLS) certification
- Proficiency in written and verbal communication
- skilled use of word processing and computer systems
- Ability to adhere to comprehensive documentation standards
- demonstrate excellent interpersonal skills
- Must be able to lift 50 lbs
- Must be available for weekend shifts
- Valid driver's license
