Job Summary
Conduct comprehensive assessments of new patients upon admission to determine unique needs and formulate customized therapy plans. Administer hands-on therapeutic interventions to enhance mobility, alleviate pain, and support functional independence. Regularly track and monitor resident progress, adjusting treatment strategies to align with rehabilitation goals. Educate residents and families on rehabilitation exercises, home care strategies, and techniques to ensure sustained progress. Document treatment plans and patient progress using electronic medical records (EMR) such as WebPT to maintain clear communication with the interdisciplinary team. Manage a patient load of up to seven patients daily within a multidisciplinary team environment.
Required Qualifications
- A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
- An active Washington State license to practice as a Physical Therapist
- A clean professional record with no disciplinary actions
Desired Qualifications
- A minimum of two years of clinical experience
- Proficiency in manual therapy techniques
- Experience with EMR systems (particularly WebPT)
- Two years of hands-on clinical experience in a rehabilitation or skilled nursing setting
- Familiarity with treatment approaches specific to the geriatric population
- Knowledge of contemporary rehabilitation practices
