Job Summary
Conduct comprehensive patient evaluations by reviewing medical histories, performing specialized assessments, and documenting findings. Develop personalized treatment plans in collaboration with patients and families, then implement and adapt protocols based on progress and evolving healthcare needs. Engage in interdisciplinary consultations to adjust care plans and complete detailed discharge planning with accurate, timely documentation. Manage personal productivity for team meetings and appointments while contributing to departmental goals and quality improvement initiatives. Pursue ongoing professional development and serve as a clinical instructor for interns. Work onsite at our modern rehabilitation facility in Springfield, IL, serving a diverse population with orthopedic injuries, post-surgical recovery, neurological conditions, and chronic pain management.
Required Qualifications
- Bachelor's degree in Physical Therapy from an accredited institution
- High School Diploma or GED
- State licensure as a Physical Therapist
- Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association
- genuine desire to learn and grow within the field
