Job Summary
Conduct personalized physical therapy treatments to prevent disability, relieve pain, and enhance physical function following established plans of care. Oversee treatment implementation by rehabilitation assistants while performing thorough diagnostic evaluations and assessing home environments for modifications. Provide prescribed services in the home including therapeutic exercises, traction, and massage therapy, and document clinical records accurately. Collaborate with interdisciplinary teams during case conferences and train patients and families on device usage. Work onsite Monday to Friday in a collaborative facility serving diverse populations ranging from post-operative to chronic conditions.
Required Qualifications
- A Bachelor's Degree from an accredited physical therapy program
- A valid and current PT license in the state of Texas, in good standing
- One year of relevant experience in an acute care or home care setting
- Current CPR certification
- Valid driver's license and automobile insurance for home visits
- A creative mindset with strong decision-making abilities
- A passion for clinical excellence and patient-centered care
