Job Summary
Conduct thorough initial evaluations based on patients' medical history and physical assessments to create individualized treatment plans. Implement evidence-based treatment protocols in line with patient evaluations, adjusting when necessary based on ongoing assessments. Maintain meticulous patient records that document the course of care, treatment progress, and goals while ensuring compliance with all state and federal regulations. Engage in effective communication with patients, families, and other healthcare providers about patient needs, progress, and recommended changes to care strategies. Foster a hospitable environment by promoting patient confidence and cooperation through empathy and active listening. Participate actively in facility meetings to align individual and organizational goals and share best practices with colleagues. Work primarily onsite in patients' homes, focusing on diverse orthopedic patients requiring rehabilitation after surgery, injury, or chronic conditions.
Required Qualifications
- A Doctorate degree in Physical Therapy (DPT)
- A valid state physical therapy license
- Current certification in American Heart Association's Basic Life Support (BLS) for Healthcare Providers
Desired Qualifications
- At least two years of experience working as a Physical Therapist
