Job Summary
Conduct comprehensive evaluations of patients with functional limitations to establish individualized treatment plans, utilizing therapeutic techniques and modalities to address evolving needs. Maintain accurate documentation of evaluations, protocols, and discharge plans while educating patients on home exercises. Engage in interdisciplinary collaboration with multidisciplinary teams and supervise support staff to ensure cohesive care. Participate in community outreach and quality improvement processes to enhance service delivery. Work onsite with a structured 40-hour schedule, collaborating with occupational therapists and nursing staff in a collaborative environment focused on evidence-based rehabilitation.
Required Qualifications
- Bachelor's Degree in Physical Therapy from an accredited institution
- A valid State License in Physical Therapy
- Basic Life Support (BLS) certification
- Proficiency in both verbal and written communication
- Strong computer skills
- Experience with electronic health record (EHR) systems
Desired Qualifications
