Job Summary
Conduct comprehensive physical evaluations by assessing medical history, performing physical tests, and determining individualized treatment plans tailored to each patient's needs. Effectively communicate assessment findings and treatment plans to patients and families while implementing protocols aligned with best practices. Collaborate with healthcare professionals to document condition changes, oversee discharge planning, and manage caseloads to meet productivity expectations. Work predominantly with diverse adult patients recovering from injuries, surgeries, or chronic conditions in our state-of-the-art Jacksonville rehabilitation center.
Required Qualifications
- Bachelor's Degree in Physical Therapy or a related field
- A valid Physical Therapist license in the state of Florida
- American Heart Association's BLS for Healthcare Providers CPR certification
- Demonstrable clinical knowledge and skills appropriate for treating a diverse patient demographic
Desired Qualifications
- A minimum of 2 years of experience in physical therapy, preferably in an outpatient or rehabilitation setting
- Familiarity with electronic medical records (EMR) systems and documentation practices
- Experience working collaboratively within a multidisciplinary health care team
