Job Summary
Conduct thorough evaluations of patients by reviewing medical histories, performing assessments, and documenting findings to develop personalized treatment plans. Implement tailored treatment protocols while continuously assessing progress and adjusting approaches based on patient feedback. Effectively communicate evaluation results and goals to patients and families, ensuring their active involvement in rehabilitation. Collaborate with interdisciplinary team members including physicians and occupational therapists to optimize care trajectories. Engage in ongoing outcome assessment and participate in departmental meetings and continuous quality improvement initiatives. Work with diverse populations ranging from pediatric to geriatric patients recovering from surgeries, injuries, and chronic conditions.
Required Qualifications
- A Bachelor's Degree in Physical Therapy from an accredited institution
- Active state licensure or eligibility to obtain licensure in Connecticut
- Strong interpersonal and communication skills, with an ability to build rapport with patients and their families
- Commitment to continuing education and professional growth, with a focus on innovative treatment methodologies
