Job Summary
Conduct thorough evaluations and functional assessments of patients to identify needs and establish treatment goals. Develop, implement, and supervise individualized treatment plans tailored to unique patient requirements, including post-surgical recovery, orthopedic injuries, neurological disorders, and chronic pain management. Provide education and guidance to patients and families regarding treatment options and home exercise programs while maintaining detailed documentation of progress in compliance with regulatory standards. Collaborate closely with interdisciplinary healthcare professionals to ensure continuity of care and participate in community outreach programs promoting physical therapy locally. This full-time role requires a 40-hour weekly commitment with occasional weekend availability at our Port Charlotte facility, serving a diverse population from young adults to seniors.
Required Qualifications
- A Bachelor's Degree in Physical Therapy from an accredited institution
- A valid State License in Physical Therapy
- Basic Life Support (BLS) certification
- Strong written and verbal communication skills
- Proficiency in word processing and electronic health record systems
- occasional weekend availability
Desired Qualifications
- minimum of 2 years of relevant experience in a clinical setting, working directly with patients to rehabilitate and improve their functional capabilities
- Experience with a variety of therapeutic modalities
