Job Summary
Implement care plans established by RPT and physicians, screen residents quarterly, carry out treatments, and monitor progress. Responsibilities include coordinating discharge planning, supervising aides, documenting care, and educating staff and families. The role requires effective communication and organization skills, and maintaining knowledge of policies and procedures. A sign-on bonus of $5,000 is offered for direct applicants.
Required Qualifications
- Associate degree in Physical Therapy Assistant program from an accredited institution
- Certification and licensure requirements as established by state and national board associations
- Active state license required
- Excellent organizational and time management skills
- Excellent written and verbal communication skills
- Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines
- Completion of all required screenings, certifications, and licensure
