Job Summary
Evaluate functional needs and outcomes, then develop, implement, and adjust individualized plans of care based on patient progress. Provide direct patient care to help individuals regain independence, improve mobility, and enhance quality of life. Collaborate with interdisciplinary team members, consult with specialists as needed, and direct therapy assistants to achieve patient goals. Maintain accurate documentation including evaluations, treatment plans, progress notes, and required signatures. Serve as a clinical resource and advocate for physical therapy within the community. Travel up to 50% within the Easton/Brockton coverage area.
Required Qualifications
- Graduate of an APTA-approved Physical Therapy program
- Current, unrestricted Physical Therapist license in the state of practice
- Minimum of one (1) year of experience in a healthcare setting
- Ability and willingness to travel up to 50% within the assigned coverage area
- Reliable transportation
- Valid driver's license
- Current auto insurance in compliance with state laws
