Job Summary
Provide in-home, one-on-one physical therapy to older adults in private residences and senior living communities, delivering therapy to improve mobility, independence, and quality of life. Utilize the Geriatric House Calls model, document in FOX’s EMR per regulatory standards, and collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists). Opportunities for telehealth/virtual care to extend access. Driving to patient homes as part of the in-home and senior living care model; flexible schedule with autonomous practice within a clinician-operated, collaborative environment.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
