Job Summary
Provide one-on-one physical therapy to older adults in-home or in senior living settings within FOX Rehabilitation's Geriatric House Calls model, delivering in-home and community-based PT services with autonomy over scheduling. Treat patients in private residences and senior living communities, potentially incorporating telehealth to extend access. Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists), document in FOX’s EMR per regulatory standards, drive to patient homes, and support mobility, independence, and quality of life. Roles include Full-time in-home/senior living care (salaried with benefits), Part-time with benefits eligibility, and PRN Home & Community Visits, with H1B sponsorship available for qualified clinicians and mentorship programs for new graduates.
Required Qualifications
- Licensed Physical Therapist in state(s) of practice
- New Graduates eligible for mentor program (Emerging Professionals)
- Degree from an accredited Physical Therapy program
