Job Summary
Conduct comprehensive evaluations of patients to establish physical capabilities and limitations, while setting SMART goals for their recovery. Design and implement evidence-based, individualized treatment plans incorporating therapeutic modalities, and educate patients and caregivers on exercises and lifestyle modifications. Collaborate closely with referring physicians, PTAs, and other healthcare professionals to ensure continuity of care, and maintain precise documentation of patient progress and clinical encounters. Perform onsite home visits Monday through Friday from 9 AM to 5 PM.
Required Qualifications
- Valid Physical Therapist license in the state of Florida or be eligible for licensure
- Bachelor's Degree in Physical Therapy from an accredited institution
Desired Qualifications
- At least 2 years of outpatient clinical experience
- Prior experience in home health settings
