Job Summary
Conduct thorough evaluations of patients by reviewing medical histories and performing specific assessments to determine diagnoses and treatment plans. Implement evidence-based treatment protocols tailored to each patient's assessment findings and current physical capabilities, adapting approaches as needed. Clearly communicate evaluation findings and treatment plans to patients and their families, collaboratively setting achievable goals based on individual needs. Prepare detailed discharge plans and follow-up care instructions, ensuring timely documentation of all care activities, including evaluations, treatment notes, and progress summaries. Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care, acknowledging and documenting any changes in patient conditions that require adjustments to treatment plans. This role focuses on outpatient rehabilitation for post-operative patients, sports injuries, and chronic pain. Candidates must hold a Doctorate degree in Physical Therapy (DPT) and a valid Virginia license.
Required Qualifications
- Doctorate degree in Physical Therapy (DPT) from an accredited program
- A valid physical therapy license in the state of Virginia or the ability to obtain one prior to starting
- Current Basic Life Support (BLS) certification from the American Heart Association
- Excellent interpersonal and communication skills
- Strong analytical skills with a proven ability to assess and adjust treatment plans based on patient progress
Desired Qualifications
- A minimum of two years of experience in a similar outpatient setting
- Demonstrated experience with electronic health record (EHR) systems
