Job Summary
Conduct comprehensive evaluations by reviewing patient histories, performing assessments, and documenting findings to develop tailored treatment plans. Implement therapeutic protocols based on individual needs while communicating results and goals to patients and families. Collaborate with interdisciplinary teams to modify care plans as conditions change and execute discharge planning with clear documentation. Maintain productivity schedules, engage in marketing efforts, and pursue continuing education to uphold clinical standards. Work onsite Monday through Friday in a supportive clinic environment serving patients recovering from surgeries, managing chronic conditions, or rehabilitating after injuries.
Required Qualifications
- Bachelor's degree in Physical Therapy from an accredited institution
- A high school diploma or GED
- Active physical therapy license for the state of Florida
- Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association
Desired Qualifications
- A minimum of 1-2 years of experience in an outpatient physical therapy setting
- Experience in home health care services
- Fluency in electronic health records (EHR) systems
