Job Summary
Conduct comprehensive patient evaluations to assess physical condition and develop individualized treatment plans utilizing manual therapy, therapeutic exercises, and modalities. Implement and adjust sessions to address specific recovery goals while documenting progress meticulously in the electronic health record system. Provide education to patients and caregivers regarding injury prevention and home exercise strategies, regularly reassessing outcomes to modify care as necessary. Work onsite with a diverse patient population including children, adults, and seniors across Jacksonville, Florida, engaging in interdisciplinary discussions to enhance patient outcomes.
Required Qualifications
- A Bachelor's degree in Physical Therapy from an accredited institution
- A Doctorate in Physical Therapy (DPT)
- A valid State License in Physical Therapy
- certification to practice
- Strong written and verbal communication skills
- Proficiency in clinical documentation
- Proficiency in EHR systems
- basic computer skills
