Job Summary

Collaborate with interdisciplinary teams to modify care plans based on health status changes and document all interactions, including daily notes and discharge summaries. Manage time effectively to meet productivity standards and engage in initiatives to enhance patient access and experience. Pursue continuing education and mentor students as a clinical instructor to support professional development.
Required Qualifications
- A Bachelor's Degree in Physical Therapy
- A valid state license to practice as a Physical Therapist in Connecticut
Desired Qualifications
- A Doctorate in Physical Therapy (DPT)
- A minimum of two years of relevant experience in a clinical setting
- Experience with electronic health records (EHR)
