Job Summary
Conduct comprehensive evaluations by reviewing patient histories and performing specific assessments to formulate effective treatment plans. Implement targeted treatment protocols tailored to each patient's unique needs and continuously adapt these protocols based on the patient's progression. Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care while documenting any significant changes in conditions. Oversee discharge planning, ensuring that patients receive adequate follow-up care. Maintain precise and timely documentation of all patient interactions and treatment outcomes, including daily encounter notes and progress summaries. Actively participate in community outreach and marketing events to promote our services. This outpatient role serves a diverse patient population ranging from post-operative individuals to those seeking rehabilitation from acute or chronic injuries. Candidates must hold a Bachelor's degree in Physical Therapy, current Florida licensure, and mandatory BLS certification.
Required Qualifications
- Bachelor's degree in Physical Therapy from an accredited institution
- Current licensure to practice physical therapy as required by the state of Florida
- Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association
- Strong communication skills to facilitate effective interaction with patients, families, and healthcare colleagues
