Job Summary
Conduct thorough patient evaluations, including medical history reviews, to devise personalized treatment plans. Implement and adjust treatment protocols based on continual progress assessments while communicating strategies to patients and families. Collaborate with healthcare professionals, document discharge planning, and maintain comprehensive records of daily encounters and progress summaries. Manage productivity expectations and participate in community outreach workshops to enhance public awareness. Work onsite at our rehabilitation center in Jacksonville with a diverse patient population recovering from surgery, sports injuries, and chronic conditions.
Required Qualifications
- Bachelor's Degree in Physical Therapy
- High School Diploma or GED equivalent from an accredited Physical Therapy program
- Valid state license
- Certification in BLS for Healthcare Providers from the American Heart Association
Desired Qualifications
- Experience in a clinical setting, particularly in outpatient therapy
