Job Summary
Conduct thorough evaluations to assess patients' physical conditions, including reviewing medical histories and administering specific assessments to develop tailored treatment plans. Implement and adjust treatment protocols based on ongoing assessments of patient progress, utilizing therapeutic exercises and modalities to optimize functional mobility. Collaborate with healthcare professionals to provide integrated care, document progress notes, and manage discharge planning for smooth post-rehabilitation transitions. Communicate clearly with patients and families regarding evaluation findings and proposed treatment strategies while maintaining a consistent Monday-to-Friday schedule.
Required Qualifications
- Doctorate in Physical Therapy (DPT) from an accredited institution
- Valid state license to practice as a Physical Therapist in New Jersey
- Current certification in Basic Life Support (BLS) for Healthcare Providers as mandated by the American Heart Association
