Job Summary
Conduct comprehensive patient evaluations using standardized tools to determine functional limitations and treatment objectives. Develop, implement, and adjust individualized rehabilitation programs including manual therapy, exercise programs, and patient education to enhance mobility and reduce pain. Collaborate with an interdisciplinary team to integrate therapeutic approaches while educating patients and caregivers on home exercise strategies and preventive measures. Ensure compliance with legal standards through meticulous documentation of all patient interactions and treatment progress. Meet specific utilization targets and contribute to operational success by enhancing clinic workflows. This role serves a diverse orthopedic, pediatric, and sports medicine population across Aurora, Illinois, within a supportive home health setting.
Required Qualifications
- A Bachelor's degree in Physical Therapy from an APTA or CAPTE-accredited institution
- A valid State Physical Therapy license
- Current Basic Life Support (BLS) certification from the American Heart Association
- Excellent written and verbal communication skills
- Strong computer proficiency
- A proven ability to build meaningful and trusting relationships with patients from diverse backgrounds
- A strong commitment to community engagement, professionalism, and continuous learning
