Job Summary
Physical Therapist will provide outpatient physical therapy services in an ambulatory setting within Odom Rehab, focusing on geriatric rehabilitation and services delivered in accordance with state guidelines. Responsibilities include evaluating patients, establishing and updating Plans of Care (POC), supervising the PTA, delivering manual therapy and therapeutic exercises, educating patients and families, documenting daily treatment, coordinating with physicians, developing discharge plans, and contributing to ongoing protocol updates. The role emphasizes adherence to OHW core values, HIPAA, continuing education/licensure maintenance, and participation in staff meetings. The position supports both full-time and part-time schedules in Waconia, MN, within assisted living facilities and surrounding community settings.
Required Qualifications
- Graduation from an accredited Physical Therapy program with a degree in Physical Therapy and current MN State Physical Therapy Licensure
- One (1) year of experience as a Physical Therapist (preferred)
- Recent / previous work experience in primary health care setting (preferred)
- Ability to follow written and oral directions
- Computer skills required
- Demonstrates a high level of personal and professional accountability and responsibility
- Self-directed and performs duties independently
- Ability to problem solve and apply critical thinking skills
- Must have the proven ability to maintain confidentiality
- For Mandatory Administrative Drivers: Valid Minnesota driver’s license, acceptable driving record, and access to a reliable vehicle required
- Must maintain personal vehicle auto insurance coverage
