Job Summary
Conduct thorough evaluations including patient history reviews and specific assessments to formulate tailored treatment recommendations. Clearly explain results to patients and families to set individualized goals, then implement and adjust treatment protocols based on ongoing assessments. Collaborate with healthcare peers to address condition changes, manage discharge planning, and maintain comprehensive progress records. Work onsite in various patient homes to deliver personalized therapy for those recovering from surgeries, injuries, and chronic conditions. Utilize time management strategies to meet productivity standards while supporting departmental goals and compliance.
Required Qualifications
- A Bachelor's or Doctorate degree in Physical Therapy from an accredited program
- A valid Physical Therapy license in the state of New Jersey
- Current certification in Basic Life Support (BLS) from the American Heart Association
- Thorough understanding of physical therapy practices
- Commitment to continuous learning
- This position is primarily onsite
- Operating within the home care setting
Desired Qualifications
- A minimum of two years of experience in a clinical setting, particularly in home care or outpatient services
