Full Time Ocupational Therapist

Occupational Therapist needed for on-site care at Holton Manor in Wisconsin. Responsibilities include conducting initial patient evaluations, developing and revising plans of care, documenting treatment, coordinating discharge with the COTA, supervising COTA coverage, and ensuring adherence to state/federal guidelines. Role involves on-site assessment and re-evaluation of patients, applying therapy modalities, and providing supervision as required by state practice acts. Requires OT degree from an accredited program and active state licensure, with experience in outpatient, ALF/ILF, or LTC settings. Strong administrative, communication, and documentation skills, plus proficiency with EMR and payer-source requirements. Flexible scheduling with full-time, part-time, or PRN options, and a collaborative, patient-centered care approach in a 60-bed skilled nursing/community rehab environment.
