Job Summary
Occupational Therapist (OT) needed at Valley View Healthcare Center (CommuniCare Rehab) for a Full Time role. Responsibilities include evaluating residents' occupational needs, developing and implementing individualized treatment plans, supervising COTAs, and leading interdisciplinary meetings to communicate clinical findings and recommendations. Opportunity to provide therapy across multiple locations within CommuniCare facilities. Requires a Bachelor's or Master’s Degree in OT and a valid or pending state OT license. Strong emphasis on resident-centered care, education of therapy staff, and collaboration within a rehab team in a post-acute, skilled nursing setting.
Required Qualifications
- Bachelor's or Master’s Degree in Occupational Therapy from an Accredited Program
- State OT license (valid or pending)
- Ability to perform patient evaluations and develop a treatment plan
- Ability to implement treatment plans and educate staff
- Supervision of COTAs
- Interdisciplinary collaboration and communication with rehab team
- Experience in post-acute care / skilled nursing settings is a plus
