Full-time - Occupational Therapist

Job Summary
Occupational Therapist screens, evaluates, re-evaluates, discharge plans, and revises plans of care under physician supervision; develops and implements treatment plans for residents with identified functional declines; reassesses caseload every 30 days; supervises Certified Occupational Therapy Assistants and adjusts plans as needed; conducts discharge planning to maximize independence in the least restrictive environment; educates staff, family, and caregivers; identifies and coordinates equipment needs and program development; completes required documentation and coordinates in-services to ensure safe carryover of Maintenance/Restorative programs; promotes the profession within the facility and participates in continuing education.
Required Qualifications
- Bachelor's, Master’s or Doctorate degree in occupational therapy from an accredited program
- Certification and licensure requirements as established by state and national board associations
- Active state license
- Excellent organizational and time management skills
- Excellent written and verbal communication skills
- Current working knowledge of Functional Pathways policies and procedures
- Completion of all required screenings, certifications, and licensure. Screening requirements may vary by location.
