Job Summary
In-home annual health assessments for Medicare Advantage and other plan members conducted by a Nurse Practitioner within Optum HouseCalls. Perform comprehensive in-home evaluations including medical history review, medication reconciliation, vital signs, physical examination, evidence-based screenings, and point-of-care testing as appropriate. Document diagnoses for care management, communicate findings to members’ primary care providers to close gaps in care, identify urgent situations, educate members on disease processes and medications, address social determinants of health, and provide referrals as needed. Role emphasizes education, collaboration with PCPs, and non-prescribing, field-based care aimed at improving health outcomes and accessibility of care to patients in home settings.
Required Qualifications
- Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care (NP)
- Current NCCPA certification and state licensure (PA)
- Active, unrestricted licensure in the practicing state (or ability to obtain by start date)
- Active and unrestricted driver's license
- Access to reliable transportation for home visits
- Ability to navigate varied home environments; ability to sit/stand/kneel as needed
- If applicable, independence as NP/PA per state practice regulations and board eligibility/approval timelines
