Job Summary
Conduct health assessments, develop personalized care plans, and evaluate geriatric patients in private homes, assisted living, and independent living facilities. Adjust treatment plans as needed, perform minor procedures and wound care, and educate families on disease prevention and lifestyle modifications. Coordinate care with physicians and other professionals while utilizing a mobile-based EHR with AI functionality. Schedule includes four or five days with minimal on-call for phone triage.
Required Qualifications
- Ohio NP license
- DEA
