Full-Time Nurse Practitioner - Geriatric Care - New Grads Welcome

Conduct health assessments, develop personalized care plans, and regularly evaluate patient conditions for a panel of geriatric patients in private homes and long-term care facilities. Perform minor procedures and wound care as required, educate families on health management, and coordinate care with physicians and other professionals. Adjust treatment plans based on patient responses and utilize a mobile-based EHR with AI functionality to reduce administrative time. On-call is phone triage only with one night per month. This practice delivers primary care without the challenges of traveling to an office, serving a team of over 40 providers across the greater Cleveland area.
