Job Summary
Nurse Practitioner providing skilled nursing assessment, planning, and home-based patient care in Albuquerque, NM. Responsible for determining patient eligibility for care based on state/federal guidelines, initiating intervention with interdisciplinary team, coordinating admission or recertification for care, and reassessing eligibility and plan of care through regular home visits. Requires NP licensure in the practice state with 1-2 years of experience, CPR certification, ability to perform home nursing visits, and strong communication with patients, families, and the interdisciplinary team. Required Qualifications
