Job Summary
Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures. Assist with activities of daily living, supervise nursing assistants, and monitor physical and emotional status to notify physicians of changes. Develop individualized care plans, complete clinical documentation, and coordinate admissions, transfers, and discharges. Participate in quality assurance, staff education, and regulatory surveys. Requires current active LPN/LVN license, minimum one year clinical experience, and familiarity with EHR systems. Full-time night shift position at Hillcrest & Hermitage Care & Rehab offers a $5,000 sign-on bonus and $8/hour differential.
Required Qualifications
- Current, active LPN/LVN license in the state of practice
- Minimum one (1) year of clinical nursing experience
- Knowledge of Federal and State long-term care regulations
- Familiarity with electronic health records (EHR) systems
- Proficiency with Microsoft Office applications
