Job Summary
Conduct thorough evaluations and assessments of patients to determine physical capabilities and rehabilitation needs. Develop, implement, and regularly update individualized treatment plans addressing patient-specific goals. Provide education to patients and families regarding health management, safety practices, and recovery exercises. Document all patient care aspects in compliance with legal standards while collaborating continuously with physicians and nursing staff to optimize recovery paths. Participate in community outreach programs and attend regular staff meetings to enhance clinical skills. Work onsite primarily in home care settings with diverse populations including elderly individuals recovering from surgery and those with chronic health issues.
Required Qualifications
- A Bachelor's or Doctorate in Physical Therapy from an accredited institution
- A valid state license in Physical Therapy
- Current BLS certification
- Excellent communication skills, both verbal and written
- Proficiency with electronic medical records (EMR) and word processing software
- Strong organizational skills to manage multiple patient cases simultaneously
- Prior experience in home care settings or community-based rehabilitation
- Familiarity with contemporary therapeutic modalities and techniques
