Job Summary
Conduct thorough physical therapy evaluations for children and adults, developing and implementing personalized treatment plans that adapt to patient progress. Regularly monitor and re-evaluate patient status, documenting clinical signs in the electronic records system via SOAP notes and progress summaries. Educate patients and families on the rehabilitation process while collaborating closely with multidisciplinary teams to ensure holistic care for clients recovering from surgeries, managing chronic conditions, or seeking wellness support. Work onsite in client homes Monday through Friday from 8 AM to 3 PM.
Required Qualifications
- A Bachelor's degree or higher from an accredited Physical Therapy program
- Master's or Doctorate in Physical Therapy
- Current and unrestricted licensure issued by the Board of Physical Therapy to practice in Washington State
- A willingness to perform physical therapy evaluations and treatments in-person as well as through telehealth
- A strong commitment to working with a diverse range of patients and adapting strategies to meet varied needs effectively
- Demonstrate exceptional collaboration, being a team player who thrives in a dynamic healthcare setting
Desired Qualifications
- Experience in home health care or a similar setting
- Familiarity with electronic health record systems and documentation practices
- Previous exposure to interdisciplinary collaboration in therapy services
