Job Summary

Conduct detailed evaluations of patients' physical capabilities and develop personalized treatment plans. Administer skilled therapy services including therapeutic exercises, manual therapy, and modalities to alleviate pain and promote healing. Regularly monitor and document patient progress while adapting strategies to optimize recovery. Work closely with a multidisciplinary team to deliver holistic care and educate patients and families on home care strategies and preventative measures. Implement comprehensive fall prevention programs and safety protocols to safeguard patients in their home environments. Maintain ethical practices, uphold patient confidentiality, and ensure the highest standards of care are consistently met. Schedule is Monday to Friday, 9 a.m. to 5 p.m. in Lowell, Massachusetts.

Required Qualifications

  • A degree in Physical Therapy from an accredited institution, stating a Bachelor's, Master's, or Doctorate level.
  • A valid and active state license to practice as a Physical Therapist.
  • Strong communication and interpersonal skills, allowing effective interaction with patients, families, and fellow healthcare professionals.
  • Proficiency in using electronic medical records (EMRs) to document treatment processes and patient interactions accurately.
  • A thorough understanding of evidence-based practices and the ability to incorporate the latest therapeutic methods into patient care.
  • A commitment to promoting patient-centered care and encouraging independence among clients.
  • Demonstrated capability to both work independently and engage effectively as part of a healthcare team.

Desired Qualifications

  • Previous experience working with geriatric populations.
  • Experience in creating fall prevention strategies in home care settings.
  • Knowledge of community resources available for elderly patients and their caregivers.
