Job Summary
Conduct comprehensive assessments to evaluate patients' physical conditions and formulate individualized treatment plans based on their goals. Implement therapeutic modalities, hands-on techniques, and tailored exercise regimens to enhance mobility and promote recovery. Provide patient education on injury prevention and rehabilitation protocols while continuously monitoring progress to adjust care plans. Collaborate with physicians and other healthcare professionals to ensure a multidisciplinary approach to patient care. Maintain meticulous documentation of patient records and clinical outcomes to meet compliance standards. Serve individuals recovering from surgeries, those with chronic pain, and seniors with mobility issues in their residences.
Required Qualifications
- Bachelor's degree in Physical Therapy from an accredited institution
- Current and valid state licensure to practice as a Physical Therapist
- A minimum of one year of clinical experience with a demonstrated ability to develop and implement effective treatment plans
- Strong foundational knowledge in advanced physical therapy techniques and modalities
- Excellent communication skills to engage effectively with patients, families, and a multidisciplinary team
- Analytical skills to assess patient needs and outcomes critically
- Experience in home health or community-based therapy settings
- Familiarity with electronic health records (EHR) systems to maintain patient documentation
- Background in specialized areas such as geriatrics or sports rehabilitation
Desired Qualifications
- Board certification by the American Physical Therapy Association (APTA)
