Job Summary
Evaluate patients' physical conditions through initial assessments and ongoing evaluations to create effective treatment plans. Deliver individualized therapy interventions, focusing on strengthening exercises, manual therapy, and rehabilitation techniques. Educate patients and their families about therapeutic exercises and strategies for ongoing care and improved mobility. Maintain accurate documentation of patient care and progress using EMR software such as WebPT. Collaborate with healthcare providers to ensure a multidisciplinary approach and modify treatment programs based on patient progress. Work with clients recovering from surgeries, sports injuries, and chronic pain conditions in a home care setting in Bridgeport, CT. Monday to Friday from 9 am to 5 pm.
Required Qualifications
- Bachelor's Degree in Physical Therapy or Doctorate in Physical Therapy (DPT) from an accredited program
- Current and valid Physical Therapist license in Connecticut with a clean professional history free from sanctions
- Exceptional understanding of orthopedic rehabilitation principles and manual therapy techniques
- Proficient in utilizing electronic medical records systems, particularly WebPT, for efficient and accurate documentation
- Ability to develop and implement individualized exercise programs tailored to patient needs
- Ongoing knowledge of current evidence-based physical therapy practices and methods
- A minimum of 2 years of hands-on experience in an outpatient or home health physical therapy setting
- Familiarity with managing a varied patient caseload
- A proven record of successfully enhancing patient outcomes through inventive treatment strategies
