Job Summary
Conduct thorough evaluations of patients' physical conditions and functional capabilities to establish a baseline for targeted therapies. Formulate individualized treatment plans, utilizing hands-on manipulation, targeted exercises, and modalities like electrical stimulation or heat applications to promote healing. Provide education on body mechanics and lifestyle modifications, while regularly monitoring progress to adjust interventions. Collaborate with physicians and occupational therapists to ensure an integrated approach to care. Maintain accurate patient records documenting outcomes and progress. This full-time role includes a Monday to Friday schedule from 9 AM to 5 PM within the home care setting.
Required Qualifications
- Bachelor's or higher degree in Physical Therapy from an accredited institution
- Current state licensure to practice as a Physical Therapist
- Board certification by the American Physical Therapy Association (APTA)
- Minimum of one year of clinical experience in physical therapy
- Proficient in advanced therapeutic techniques and modalities
- Demonstrated analytical skills to evaluate patient progress and modify treatment plans accordingly
- Exceptional communication abilities to foster positive interactions with patients and their families
Desired Qualifications
- Demonstrated success in a home care setting or outpatient rehabilitation
- Experience in utilizing telehealth technologies for patient assessments and follow-ups
- Experience working with geriatric populations as well as post-operative patients
