Job Summary
Evaluate patients referred by physicians to determine specific needs and conditions. Create individualized treatment plans tailored to diagnoses to promote recovery and progress toward functional goals. Administer therapeutic treatments including exercises, modalities, and functional training based on current research. Demonstrate excellence in patient communication and customer service while educating patients and families on exercises and wellness strategies. Collaborate with a multidisciplinary team to enhance outcomes and meet performance metrics including patient utilization goals. Work onsite Monday through Friday for 40 hours per week at our Jacksonville, Florida rehabilitation center.
Required Qualifications
- A Bachelor's Degree in Physical Therapy or higher
- A valid physical therapy license in the State of Florida
- A graduated education from an APTA or CAPTE-accredited physical therapy program
- Basic Life Support (BLS) Certification from the American Heart Association
- Strong written and verbal communication skills
- Proficiency in word processing and computer software relevant to patient care and documentation
- Work onsite within our state-of-the-art rehabilitation center located in Jacksonville, Florida
- Work schedule is Monday through Friday, with shifts spanning 40 hours per week
