Conduct comprehensive patient assessments to identify therapeutic needs and create individualized care plans targeting desired outcomes. Implement and manage treatment interventions utilizing evidence-based practices and modalities aligned with physician directives. Direct a majority of patients' physical therapy needs, ensuring continuity of care and facilitating transitions through recovery stages. Maintain consistent documentation of patient progress, modifying treatment plans as necessary to align with evolving conditions. Instruct patients, families, and caregivers on post-therapy exercises, therapeutic devices, and techniques to enhance recovery at home. Manage caseloads effectively by prioritizing daily tasks and coordinating with team members to optimize patient care. Work onsite Monday through Friday between 2 PM and 8 PM.
