Job Summary

Collaborate with interdisciplinary healthcare professionals to coordinate care, document condition changes, and manage discharge planning and follow-up processes. Tailor treatments to meet specific needs of home care patients recovering from surgeries, illnesses, or injuries. Maintain high productivity standards and ensure compliance with governmental and accreditation regulations.
Required Qualifications
- A Bachelor's degree in Physical Therapy from an accredited institution
- A valid state license for physical therapy practice in Florida
- Current BLS (Basic Life Support) certification from the American Heart Association for Healthcare Providers
- Strong communication skills
- The ability to work collaboratively with a diverse healthcare team
