Job Summary
Develop and implement tailored treatment plans based on thorough assessments for patients recovering from orthopedic surgeries, sports injuries, neurological conditions, and age-related mobility challenges. Collaborate with an interdisciplinary team including nurses and occupational therapists to provide cohesive care, while conducting educational sessions for patients and family members regarding rehabilitation practices. Attend Care Plan Meetings to share progress updates and adjust therapy strategies, maintaining accurate documentation of evaluations and interventions. Work onsite within a skilled nursing facility Monday through Friday, refining approaches with a supervising Physical Therapist to maximize recovery.
Required Qualifications
- Valid Physical Therapy (PT) license in good standing in the state of Virginia
- Bachelor's degree in Physical Therapy
- Current CPR certification
- Strong interpersonal communication skills
- A dedication to clinical excellence, integrity, and the rehabilitation process
- Ability to adapt treatment approaches based on individual patient needs and preferences
- Adherence to CMS' COVID-19 Vaccination Mandate
Desired Qualifications
- Doctorate in Physical Therapy
- Previous experience working in a skilled nursing facility
- Experience in developing multidisciplinary treatment plans in collaboration with other healthcare professionals
- Familiarity with federal, state, and regulatory agency guidelines relevant to therapy practices
- Strong ability to engage and educate patients and their families about the therapeutic process
- Demonstrated ability to utilize evidence-based practices in treatment
