Job Summary
Conduct comprehensive evaluations to assess patient physical conditions and rehabilitation needs. Develop and implement personalized treatment plans utilizing exercise regimens, manual therapy, and adaptive equipment. Track patient progress through documentation and regular assessments, adjusting plans based on individual feedback. Provide education to patients, families, and caregivers regarding techniques and home exercises. Collaborate with multi-disciplinary teams including physicians and occupational therapists to coordinate care. Stay updated with evidence-based practices through professional development workshops and continuing education. Serve a diverse population of adults recovering from surgeries, injuries, or chronic conditions, as well as children requiring rehabilitation.
Required Qualifications
- Bachelor's degree in Physical Therapy
- Current state licensure in Physical Therapy
Desired Qualifications
- Doctorate in Physical Therapy (DPT)
- A minimum of 2 years of professional experience as a Physical Therapist
- Experience in a rehabilitation facility setting
- Prior experience in home care services
