Job Summary
Conduct thorough evaluations including medical history reviews and targeted assessments to formulate individualized treatment plans with measurable goals. Implement evidence-based protocols based on assessment outcomes, adjusting as necessary for changing healthcare landscapes. Collaborate with healthcare professionals to address multidimensional patient care aspects and document condition changes requiring reassessment. Manage discharge planning and follow-up care while meticulously documenting all interactions, evaluations, progress notes, and discharge summaries. Engage in department initiatives, pursue ongoing professional development, and serve as a clinical mentor to students and newer therapists. Fulfill productivity expectations and maintain an organized schedule to ensure timely attendance at meetings and appointments.
Required Qualifications
- A Doctorate degree in Physical Therapy (DPT)
- A valid Physical Therapy license in the State of Connecticut
- Current CPR certification from the American Heart Association
Desired Qualifications
- A minimum of two years of clinical experience
