Job Summary
Assess psychosocial status of patients and families regarding terminal illness and environment, then communicate findings to the registered nurse and interdisciplinary group. Provide assessments in patient residences when safe, counsel families on stress and coping difficulties, and maintain clinical records. Carry out social evaluations, develop intervention plans, and serve as a liaison between patients and community agencies. Participate in interdisciplinary meetings, quality assessment teams, and contracts with public and private agencies. Assist physicians in understanding social and emotional factors related to death and dying. Requires an MSW, two years of hospice experience, CPR certification, and bilingual Spanish skills. Full-time role with 100% travel and on-the-road location.
Required Qualifications
- Master's program (MSW) in social work accredited by the Council on Social Work Education
- Clinical experience relevant to the psychosocial needs of patients and families
- Minimum of one year's experience in health care
- Experience in hospice care and/or facility
- Good verbal and written communication skills
- Organization skills
- Current CPR Certification
- Licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- 100% willingness to travel
- 2 years Hospice and/or Palliative Care experience
