Full-Time Hospice RN Case Manager

Job Summary
Manage an assigned hospice patient caseload and provide ongoing clinical oversight, including comprehensive nursing assessments, routine visits, and PRN visits as clinically indicated. Develop, implement, and update individualized plans of care while assessing for changes in condition, disease progression, and evolving symptom-management needs. Manage pain and other end-of-life symptoms using hospice best practices, communicate changes in condition to physicians and hospice providers, and obtain appropriate orders. Provide education, support, and guidance to patients, families, and caregivers, and coordinate care with hospice aides, social workers, chaplains, physicians, facilities, pharmacies, DME providers, and other members of the interdisciplinary team. Participate in interdisciplinary care planning and team meetings, complete accurate and timely documentation in the electronic medical record, and handle admissions, recertification assessments, and deaths as appropriate. Ensure care aligns with the hospice plan of care and regulatory requirements. Primarily weekday field-based patient care with participation in clinical coverage and/or on-call rotation as scheduled.
Required Qualifications
- Current Texas Registered Nurse license in good standing
- Minimum one year of RN experience
- Reliable transportation
- Valid driver's license
- Current automobile insurance
- Current CPR certification
