Job Summary
Initiate, develop, and implement basic nursing plan of care treatments while evaluating patient progress. Provide direct patient care in homes or alternate sites, administering medications, educating families, and modifying treatment plans based on physician orders. Assess patient needs through interviews and physical reassessments, obtaining clarifications on physician orders and providing written summaries of significant changes. Prepare clinical documentation in real-time for billing optimization and coordinate discharge planning with thorough progress reporting. Communicate effectively with the clinical team and medical providers to ensure continuity of care while adhering to all safety, regulatory, and HIPAA guidelines. Participate in multidisciplinary conferences, mentor new nurses, and fulfill on-call responsibilities as applicable.
Required Qualifications
- Active and unrestricted RN license in the state of practice
- Minimum of 0-2 years of infusion nursing experience
- Current CPR certification (in-person class required)
- Demonstrated competency in patient care standards
- Advanced certification and training as applicable
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel
- Proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to self-evaluate for annual appraisal
- Ability to jointly set professional growth goals with nurse manager
- Ability to apply knowledge in the effective use of nursing technology tools
- Ability to see up close and from a distance
- Color and peripheral vision

- Ability to lift a maximum of 75 pounds

Desired Qualifications
- Bachelor of Science in Nursing as granted by an accredited school of nursing
- 1 year of previous experience as a RN in a home health or acute care setting
