Job Summary
Design and implement individualized physical therapy interventions for pain relief, mobility enhancement, and disability prevention based on physician prescriptions. Conduct evaluations using diagnostic tests to assess muscle, nerve, and joint function, while assessing home environments to identify improvement opportunities. Deliver direct therapy services at patients' residences, applying manual techniques, therapeutic exercises, and educational support to promote rehabilitation and independence. Supervise physical therapy assistants to ensure interventions align with established care plans and collaborate in interdisciplinary meetings to discuss treatment progress. Maintain meticulous clinical records for compliance and educate clients on home therapy techniques and equipment use.
Required Qualifications
- A Doctor of Physical Therapy (DPT) degree from an accredited program
- Minimum of one year of experience as a Physical Therapist, particularly in acute care or home healthcare environments
- Current and valid state licensure for physical therapy
- CPR certification
- Valid driver's license
- Automobile insurance
- Strong commitment to delivering high-quality clinical care with integrity and empathy
- Ability to engage effectively with patients, families, and interdisciplinary teams, demonstrating strong interpersonal skills
- Proficient knowledge of federal and state healthcare regulations, including those pertaining to home healthcare settings
Desired Qualifications
- Experience with electronic health records (EHR) systems to aid in accurate documentation and reporting
- Background in developing and implementing patient care plans demonstrating measurable outcomes
