Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom management and daily care while monitoring progress to prevent avoidable hospitalizations. Coordinate care across providers to ensure continuity and complete accurate documentation supporting quality outcomes. Schedule includes Monday through Friday day time visits with a shared on-call rotation for Strum, Eleva, Osseo, Fairchild, and surrounding communities. Requires an active RN license with at least one year of clinical experience, a current driver's license, and CPR certification.
Required Qualifications
